Journey of Hope, Inc. and all Journey of Hope, Inc. Staff Members, Board Members, Volunteers, Mentors, and/or anyone representing Journey of Hope, Inc. in any capacity that would have access to ANY Client information, whatsoever, follow all Federally Mandated HIPAA Privacy & Security Standards, Guidelines, Procedures & Processes.
All Client information, specifically, protected health information & electronically protected health information is kept 100% confidential and is secured in our HIPAA Compliant CRM, and is only transmitted with the client's explicit permission and consent, in circumstances relating to Referrals between Community Partners, State or Federal Assistance applications or opened/closed cases, Healthcare Providers, Therapeutic Resources/Providers, pertaining to the Client's Case Management Needs Assessments and/or Care Plans, as well as Organizations and Community Resources with a "Memorandum of Understanding" in place (with Journey of Hope, Inc.), as well as in circumstances relating to processes and procedures for Case Management Services, Certified Peer Support Services, Mentoring Services, and other Emergency & Direct Services.
We also share may share information related to Client Cases with Legal Entities, for example, the Courts, Parole or Probation Officers, Lawyers, or other Counsel, including the initiation of legal proceedings, filing protective orders, correspondence between law enforcement agencies about the case specifically, and other legal entities related to legal procedures and processes requested by the client, with the client's explicit permission.
IF there has been a crime committed against Youth or Elders, we are required by law to report the incident to Law Enforcement, as well as any other cases related to violent and/or continued abuse of Youth or Elders (AKA Older Adults), or any other situations where harm or death may occur, including anything that could potentially impact the Liability or Legal standing of Journey of Hope, Inc., or its Staff Members, Board Members, Advisory Board Members, Committee Members, Volunteers, Mentors (Prison Mentors or otherwise), Community Partners, Funding Partners, or any person or entity that is specifically related to Journey of Hope, Inc. and our Day to Day Operations.
